Why was I charged an extra 40, I paid for state and federal, I didn't agree to anything else

It looks as though you are using TurboTax Deluxe. This version is $39.99 to file federal and $36.99 to file state.

If you choose to have your fees deducted from your return there is an additional $39.99 fee.
