Why is one of my children's name at the top of my 1040 form on the same line as filing status?

This indicates that a nondependent child qualifies you for the Head of Household filing status.

Why would this prevent the other parent from filing this child as a dependent?  If I did NOT use this child as a dependent, but only for child care taxes?

No, the other parent can claim, as long as you are separated or divorced.

Why is one of my children's name at the top of my 1040 form on the same line as filing status?

His tax person is stating that I have claimed this child.  I claimed child 1, he is claiming children 2&3.  All paperwork indicates this and looks great, for the exception that child 2 randomly shows up at the top of the 1040 next to filing status.  Why would it not be the child 1 that I did claim?

You have to go back over your entries. If the wrong child is included, change it.
