where do I enter employee 401k contributions? only listed on w-2 are the

Box 12a - 12d Box 12 - Code D - Elective deferrals to 401(k) Box 12 - (shows what employer matched)

W - Employer contributions to Health Savings Account Add another box 12 item( shows what employer paid)

Your contributions to your 401(k) account are reported in box 12 code D of your form W-2 and the amount has already been subtracted from taxation in box 1.

You do not need to need them anywhere else on tax return.
