When entering "Amount paid" for child care for our nanny, do I include just gross wages or also include Household Employment taxes from Schedule H?

Yes, you can include the employment taxes you paid on the nanny's wages as part of your child care expenses.  According to the IRS Child and Dependent Care Expenses instructions for Form 2441 page 2:

"You can also include your share of the employment taxes paid on wages for qualifying child and dependent care services."
