The casino should have had you both sign and date Form W-2G. They then would have given you copies for your records and tax filing.
If this didn't happen, and if you have already contacted the reservation requesting a copy, you may need to contact the IRS directly to ensure you do everything you possibly can to obtain this information.
You can also try checking your account with the IRS to see if the withholdings from the winnings are reported by requesting a tax transcript.
When you prepare your tax return, you will still need to include all winnings to ensure all of your income is reported. Please see this link for more guidance on reporting winnings and losses. If you have records to support your winnings and withholdings, keep those in a safe place in the event the IRS reaches out for more support.
