W2 form missing 12 box W even though I contributed?

I have a 5498-SA showing total contributions to HSA in 2024 was $1,440.00 but I have no W code in my 12 box on my W-2. Should I ask for a corrected W-2 or just not worry about reporting contributions? I did all my contributions through my employer.

You must worry about reporting contributions - they are tax-free, after all..

 

First, the code W shows the total amount that your employer and you (through payroll deduction) contributed to your HSA. It does not show HSA contributions you made directly to your HSA custodian.

 

Box 2 on the 5498-SA shows TOTAL contributions, which includes contributions through your employer (the code W amount) AND any contributions you sent directly to your HSA. So it is possible that there should not be a code W on your W-2 - how DID you make HSA contributions in 2024?

 

So, you can do the following with your employer:

1. Ask your employer if they made any HSA contributions - either by them or by you through payroll deduction (or both).

2. If the answer is Yes, then ask them if they removed this amount from Wages in boxes 1, 3, and 5 on your W-2? 

3. If they in fact made HSA contributions for you and they removed them from Wages in boxes 1, 3, and 5 before they printed your W-2, then do the following:

  • Go into your HSA interview (at Deductions & Credits->Medical->1099-SA, HSA, MSA). Indicate that you have an HSA and continue. 
  • At some point, you will come to a screen that says "Did your employer tell you about any other contributions?"
  • Click on Yes.
  • Three new lines will appear.
  • The last one is "Employer and payroll contributions not reported in Box 12 of your W-2".
  • On this line, enter the amount of HSA contributions made by and through your employer.

4. If your employer didn't have anything to do with HSA contributions, then on the prior screen ("Let's enter [name's] HSA contributions"), enter the $1,440 as personal contributions on the bottom line. This will be removed from your income on Schedule 1 (1040),

 

There are other possibilities, but I will stop here to see if this works for you.
