What kind of form? And is it a Federal EIN or a state tax ID number? If latter, what state?

It’s W2 form, MN State ID number

Check to be sure you correctly indicated the correct state abbreviation in Box 15 of the W-2 entry. If a dropdown menu was used, it can sometimes move and select a different one.

And for the adjacent state tax number field in Box 15 did you use a 7-digit number?
