No. It should be on line 4 not 5. Check if the little IRA box is checked between boxes 7&8 on the 1099R. Then is should be 0 on the b line with the word rollover by it. Did you get a 1099R? What code is in box 7? Is it a G? If you didn't get a 1099R you might not need to report it.
Are you sure it was from a ROTH IRA? And not a ROTH 401K from work?
