Are you sure you have a W2? Ask your employer for a corrected one. Could it be a 1099NEC?

Do you mean those boxes are blank on your W-2? Or do you have some other tax document that is not a W-2? Do you have some kind of 1099? If so---what kind?

That is unusual. Note that the boxes are out of order on the W-2. I suggest reaching out to your payroll department and requesting a complete W-2.
