The Letter from the IRS is requesting a copy of your Health Insurance coverage document 1095-A. In regards to payments of premium tax credit if you purchased health insurance in 2017 through Healthcare.gov or your state's Health Insurance Marketplace, you'll receive a 1095-A, Health Insurance Marketplace Statement. Read the letter carefully and follow the requested instructions and provide the necessary documents to the IRS.

But where do I find the form I need when I am in the Healthcare.gov website? I am in there now but don't know where to find it

10-95A is healthcare subsidized or thru the state.
I am in il. Although disabled I get Medicare, my kids don't. But I qualify for Medicaid or what used to be Medicaid for my kids due to income.
I have chosen for them to be added to opt into the state meridian healthcare plan which is a form 1095-A form. When doing taxes if you chose a state funded plan you are on a 1095-A.
It asked me in tax section, but also said if I had a 1095-A I didn't need to add the info. I am assuming because the state has notification sent to irs. If you chose thru Obama care it's a different form, I wouldn't worry about it if your on a state funded plan. If other, like Obamacare offset, or chip, you need the form or it will time you for each person including kids. Best I know, hope it helps
