I repaid social security over $20k in 2021. they repaid me 12/31 on husband account but it isn ot included in my 2021 1099. The "net" income is now a negative but will make my social security over 40K next year... if the ssa 1099 comes in as paid in January 2022 (which is what they are telling me). What do I do to NOT get the increased medicare tax and how else will this imipact me.

You will have to pay tax on the increased amount in 2022. You should look at all your income for 2022 and see what the effect will be on next year's taxes. Then you can make decisions.
The increased amount should really only effect one year's taxes. Which happens.
if the 1099 ssa says benefits for a prior year, assuming they do not change the current 1099 ssa, then must I file an amended for 2021?
what decisions are you talking about? If they double tax next year, seems like I am stuck with it??

You will not amend your return to handle a lump-sum social security payment from a prior year. Here is the IRS reference.
As explained in the reference you will only pay taxes next year on the part of the lump sum that would have been taxable this year, if any.
TurboTax asks will ask you as part of the social security form 1099-SSA entry if you have prior year lump sum amounts. You will be asked to enter information from your prior year (Form 1099-SSA, and tax return form 1040).
