Should I answer yes or no to living in the 2 schools districts?

There are 3 schools districts on my W-2s but I only lived in 1 of the 3. I  worked in 2 of the 3 school districts. In the dates of residency for school district section, I am being asked to provide the dates of nonresidency because I selected that I did not live I the school districts. All three school districts are considered Montgomery County, Ohio.

Thank you. It also asks me for the dates of non residency, should I put 12/31/2015-12/312016?

Also, what should I put as income earned as a non resident? this amount is not labeled on my W-2s.
