Recharacterize tradtional IRA to roth IRA

I want to recharacterize $5000 IRA all to Roth IRA. What is the correct steps and what the number should i put in each step? In the recharacterization explanation, should i say the amount recharacterized and the amount transferred the same as $5000 since I want a 100% recharacterization?
