What is the beginning and the end date for COVID19 disaster Form 8915E-T?

There are no beginning and ending dates for Covid-19 because it is not a federally declared disaster.  Just enter anything and continue since the Form 8915-E is not sent with or included with a 2021 tax return.  

The Form 8915-E was only for tax year 2020.
