I can only surmise from your question that you are asking about a 1099-G for gambling winnings. If there is no date listed, enter an approximate date the winning occurred. For the most part, the IRS and state agencies are only interested in the totals but if all information is necessary to file, then an approximation, in this case a date, is acceptable.
