My bank is not recognized in the 1099-INT

Not many will be recognized.

 

Don't put any bank in the top field, then hit the "Continue" button at the bottom...then you'll get the whole 1099-INT form, and you can enter your bank there and whatever $$ are on your paper 1099-INT form.
