It will just be in box 2 of your W-2.   It is not reported in any special way.   Enter your W-2 exactly as it appears when you prepare your tax return.

No not separate.  It will all be included on W2 box 2 (box 17 for state).

Yes ... all withholding by the employer will be reported on the ONE W-2 form they will issue ... there is no separate form.
