Yes, you need to report your W-2.

You should report all your income.

I don't have access to your 2018 return but here are your options:

  • If you have a copy of you 2018 Tax return, check line 7 on the 1040 form. 
  • If you can not locate your tax return, request an online transcript from the IRS website.
  • Request printed transcript of your return on the IRS website that will be sent to you by mail to the last address on your last return.
  • You can print and mail in your return.
