I'm having trouble finding the exact answer to my issue, but there seem to be similar problems. This year my wife's employer did not list DCFSA amounts in either box 10, 12, or 14. In the past they had listed the full $5,000 we contributed in both box 10 and as code Y in box 14. How do I know if our contribution amount has already been deducted from the income reported on the W2 since it is not listed this year? Should I just complete Box 10 and Box 14 the same way it was done in the past, $5,000 and Code Y $5,000?

Do NOT change the W-2 form you have in hand ... talk to the employer and have them issue a corrected form if it is wrong.
Unfortunately the employer said they are not required by law to report this on the W2 and will not issue a new one.
I went through the credits/deductions part and TurboTax suggested I consider using an FSA in the future because there was no place to input this years contributions; only childcare expenses. I have no place to input the DCFSA deductions.
