It wants business address where the work was done...do we put the actual business address or our home address where he actually did the work?

You should use your own address as the address of your business.

Hi! It's not his business...he did work for another business at home due to covid...typically he would have done it in their office. Should I put their address or his (ours)? Thank you!

If you are entering Form 1099-NEC, presumably your husband was the recipient. The payer is the person who received the service and therefore paid the money out.
