Yes.  You will need to upgrade because you are considered an independent contractor when box 7 of the Form 1099-MISC is completed.

A Form 1040, Schedule C or C-EZ is required to report your income and any business related expenses. You may also be subject to self-employment tax if your net business income is $400 or more.
