On my filing instructions it shows a balance due of $218 but says to mail form 1040_V with a payment of $196. Why the difference and do I pay 218 or 196?

The tax form is what got transmitted to IRS, so I'd pay the amount on the 1040V.
What really matters is what's on line 22 of your Form 1040. Make your payment in that amount.
