Not able to file due to "Form 5329-s"

I'm 43, single, been using turbo tax for 8 years, most all info is exactly the same - I'm now being stopped from completing my filing due to the following message: "Form 5329-S: Your calculations could not be completed.  Reference code: 0-255-656."  This has never happened before.

From what I can research this deals with IRA issues.  I contribute the max every year but have never withdrawn a dime.  I have not received a 1099-R

The 5329 is used for many reasons depending on  what line on the form has an enter.   What line has an entry?

I have no form/never seen this form.  There is no form to inspect either on my turbo tax profile or in my actual documents.  Am I simply not seeing/locating this form in my online profile?

 Are you using online of the CD/download desktop version?

online - same as previous years.  I spent an hour on the phone w/ specialist last night... they are stumped as well.

Print all forms and worksheets and see if there is  a 5329 in the resulting PDF file.

just went through all 80 pages twice; no 5329 form anywhere.
