Non- medical Independent Companionship

I’m going to be a non medical independent companion for an independent 93 year old gentleman.  I’ll take him to his activities, appointments, help with laundry, and simple house tidying up. 

I’m prepared to withhold my own deductions, taxes, etc. 

 

Does he prepare a 1099 form at tax time?

Do I supply him with a W9 Form?
