I received two of the same W2 forms, do I still enter them?

I got two of the same identical W2 forms but one says intensionally left blank on the bottom right corner and the other one has all of the employer info on it. Do I submit both?

No.  It sounds like your employer inadvertently sent out a duplicate because of missing info.  If the income on each of them accurately reflects your income for the year, and they are identical, I would only enter one.  You may want to notify your employer and verify that your income is reported properly.
