Was the injury work related (workers comp)?

no, i was drawing sick and vacation hours to continue getting a check.

Did you sell your vacation time back and get a lump sum or did your employer let you use vacation time when your sick time was gone?

yes, i used vacation after my sick time ran out. i did not sell any time back.

Thank you for the responses to help me understand better your situation.

When you use your vacation time in a way that your employer permits and you do not sell it, you do not need to pay any additional tax on the income over and above what you already paid out of your salary when it was paid to you. 

If you had sold it back in a lump sum, that would be additional income to you.
