I made an estimated tax payment in the early part of 2016. I'm wondering if a form of some kind will be mailed to me for me to include this on my 2017 return?

No, a form won't be mailed to you.  You will include the estimated payment for your 2017 taxes as follows:

1.  Go to Federal.

2.  Select Deductions and Credits.

3.  Choose Estimates and Other Taxes Paid.

4.  Select Estimated Taxes Paid.

This is where you will add the estimated taxes paid.

No, a form won't be mailed to you.  You will include the estimated payment for your 2017 taxes as follows:
