My wife is going to do a travel nursing contract, we live in TN. If she gets a 3-month contract in a state with with income tax, do we pay income tax to that state?

Most likely yes, unless she makes less than the minimum filing requirement in that state.

She may need to make estimated tax payments as well (Federal and State).
