My wife and I got married October 2016 and she legally changed her name, but her work W2 has her old name, do I file her with her new name even if it doesn't match?

Yes, you file with the name that is registered with the Social Security office.  They send the name change to the IRS, so your wife's records will be matched with her maiden name since her social security number does not change.
