Why did your employer not withhold Federal taxes?

You should ask before you go any further with your tax return.  If it is a mistake the employer needs to give a corrected W-2.  When you got your paychecks did you notice if tax was being withheld?  Do you have some pay stubs to check?

A big one.  If no tax was withheld, then you are going to owe the IRS.

Do you remember what deductions you chose when you filled out your W-4? Like single and 0? Or married and 1? This effects your amount of taxes withheld from each check.

Hmm, then you should have had taxes withheld. Do you have access to your pay stubs to see if taxes were withheld?

Federal should be your federal taxes withheld and medical would be your pretax payment for health insurance.
