You can still file a joint tax return. You are married, even though your spouse is in a nursing home. You can still get all of the tax advantages of a joint return including the standard deduction for a married couple. You have not said how you are paying the nursing home--if you are itemizing deductions then the nursing home could provide you with a breakdown of the out of pocket costs that might be entered on your tax return as medical expenses.
