My completed tax retun has pages for 2022 540-ES Payment Form 1, Payment Form 2, and Payment Form 4. But no Payment Form 3. Were is Payment Form 3?

Per the State of California,  installments due shall be 30% of the required annual payment for the 1st required installment, 40% of the required annual payment for the 2nd required installment, no installment is due for the 3rd required installment, and 30% of the required annual payment for the 4th required installment.
