Is your spouse alive or are you needing to sign as a surviving spouse? Review the information in your My information section to make sure something didn't get changed there.
Thanks for your reply! I am filing as surviving spouse. My wife passed away in December 2025. I reviewed the My Information section and it all looks correct. I tried to e-file again but it was rejected with the same error code. It still doesn't show "Filing as surviving spouse" in the spot for Spouse's Signature on the 1040-X though.
This rejection happens because your spouse's SSN has been locked by the Social Security Administration because their records show it belongs to someone who is deceased. Verify that when you entered her information on My Info, you marked that she passed away and the date.
If the reject continues, you may print this return and mail it to the appropriate IRS Service Center.

Thanks for your response! The My Info section looks exactly as you described it, but it still keeps getting rejected.
I can print it out and mail it in but I'm concerned that it will be rejected if I don't really understand the reason for the rejection.
