Could you please try to re-enter your First and Last name in Personal Info section to make sure there is not special character or extra space coming through on consent page.

I had the same problem to make a long story short I could not get help online what I did was this I’ll use my first name as a example all keyboards are different I came up with this I put with cap lock off my keyboard still made the first t in capital letter so I put two t’s in my name
examle = Ttimothysmith without a space then use my direction key to to go back and put a space between my first and last name and go back and delete the first T that made it all small cap and it went right through I hope this works for you good luck
