It keeps asking me to fill in boxes 15-17, but only box 16 is filled in on my W-2. It won't let me proceed unless i fill all of them in. What do i do?

Enter the Employer's Federal EIN in box 15 - (see Box b at the top of the Form W-2).

Enter "0" in Box 17.

This should resolve your issue and allow you to proceed.
