If you purchased Turbotax at Costco, is there an additional charge to file both fed and state?

You will need to look at the box to see if one free state is included. Keep in mind however there is a $19.99 charge to efile a state return.

Well you get 5 free federal efiles.

For state.....Deluxe comes 2 ways, Deluxe with a free state program download and Deluxe without state for states that don't have an income tax.

Then it is $20 to efile each state return including the first one.   The early price usually expires on March 1 and goes up to $25. Or you can print and mail state for free.

But make sure you are using the Desktop program installed on your computer and you are not accidentally in the online Deluxe version.   It shows online Deluxe on your post.  If you see a black menu bar down the left side that is the Online web version not your Costco program.
