Regarding the consent to e-file a Massachusetts return for a deceased relative, do I fill in my information of that of the deceased?

If you are filing a return on behalf of the deceased (i.e. you are executor or personal representative), you would enter the deceased's information on Form 8453, 

It is the deceased's return, not yours. You may be "consenting to e-file" but it is on their behalf, not yours.
