If my Difficulty of care W-2 has $0 for federal taxes do I still submit this to the IRS when mailing?

You only need to attach W-2s or 1099-R that show withholding on them. If your Difficulty of Care W-2 doesn't have any amount in Box 2, it's not necessary to mail with your return.
