If I was married until 12/15/2016 (when my divorce was finalized), do I file married but separate or single?

I also want to know, if my legal name is <redacted> but my W2 from work still says <redacted>, will that be an issue? My employer has been taking a while to change my name.

(PII was redacted as this is a public website.) Since your divorce was final before December 31, you file as single (or head of household if you qualify). File using the name you have on record with Social Security; if you haven't changed it there, find your nearest Social Security office by visiting www.socialsecurity.gov or calling 1-800-772-1213.
