If I was married until 12/15/2016 (when my divorce was finalized), do I file married but separate or single?

I also want to know, if my legal name is <redacted> but my W2 from work still says <redacted>, will that be an issue? My employer has been taking a while to change my name.

File using the name you have on record with Social Security; if you haven't changed it there, find your nearest Social Security office by visiting www.socialsecurity.gov or calling 1-800-772-1213.
