the TT filing instructions should have the addresses

however both the IRS and most states allow payment throught their websites and generally you can set up a payment 30 days in advance (state may be different) 

just an FYI.  you can efile federal now and delay payment until 7/15

since you didn't specify the state you'll have to see for yourself when you have to pay.
