If I’m 100% North Carolina resident, but my ex-employer’s headquarters is in Mississauga, Ontario, Canda (zip code: L4W-5B5) how do I answer box C for employer address?

I was born and raised in NC. My job was as a phone vender in a North Carolina store. Can I just put in the local address?
