You cannot have any other medical plan (except the high deductible plan) and qualify for an HSA.  You and your employer need to stop contributing the same month as you start eligibility for Medicaid.  You can continue to withdraw money from your HSDA for qualified medical expenses.

All contributions must stop.  It is your account, so all contributions are "your" contributions, even if some of them come from your employer (technically, you agree to a reduction in salary, with the money put in the HSA instead, so it's all your money in the end).
