I broke your questions down into three answers -
1. If you are sure that you are going to get a refund, then you don't actually need to file an extension. The penalties associated with not filing on time are all based off taxes due. So, when you have a refund, there is no possible penalty and the IRS does not require an extension for a refund. Of course, if you are not quite positive, then its best to file an extension just to be sure.

2. You can file whenever you are ready. There is no time limit you have to wait to file or get your refund. You can even file before April 18th if you get the forms on Monday.
3. Most states give you an automatic extension with a federal extension but not all. See the list below for your particular state - it has instructions on how to proceed if there is another form you need to file.
