If I claim a dependent and she makes under $4,000 where do I list her wages, federal and state tax withheld? Do I need to enter her income at all?

You do not enter a dependent's earned income from wages reported on a W-2 on your tax return or their income from self-employment on your tax return.  The dependent can file their own tax return and indicate on their return that they can be claimed as a dependent on someone else's tax return.
