If I already am using the $119.99 service through Turbo Tax, when I e-file, will it charge me an additional 39.99?

Well yes if you do a state return.  Actually currently state is 44.99.  The 39.99 charge is if you want to have the Turbo Tax fees deducted from your refund.  So pay the fees with a credit card to avoid the extra charge.  WATCH what you click on!  Go slow and read all the screens carefully.
