Since the W2 itself does not have the Colorado portion of income reported, what you will do is prepare a worksheet that reflects what method you used to determine the Colorado income.  You don't need to send this in with your Colorado return, but keep it with your record in case Colorado asks for it.  TurboTax does not have a specific form for this.

I added the 4 months to my total CO income however it is not reflected on my W2. Are there any other forms I have to include to report this income correctly? Does turbo tax support this?
