That depends on who paid your disability payments.
If you were paid disability by the Social Security Administration, you will get a Form SSA-1099. You would need to claim that on your tax return and it may be partially taxable.
If your employer paid your disability pay, you will get a W-2 form with 3rd party sick pay on it. It will taxable.
If your disability was paid by Worker's Compensation or the Veterans Administration, it is not taxable and you will not receive a form for it.
If your disability was paid by an insurance policy that you purchased, the pay is not taxable and you will not receive a form for it.
I was paid by the employment development department (EDD).
