I was not able to get the forms I need to send in my balance due to the Federal goverment nor to CA state. How can I get the address and forms needed.

On filed my taxes using your online service.  Now I cannot get back into it to get form 1040-V to send my check to the govt.

When you sign onto your online account using the exact same user ID that you used to create, complete and file your tax returns, you should land on the Welcome Home screen.  Click on the blue link Download/print return.  The forms you need will be included in the downloaded PDF.
