I was at college in PA during the first half of 2016 and had health insurance through my university. From July - Dec I worked in MA and had health insurance through my employer with a different health insurance company. How do I report this correctly?

In the federal Health Insurance section, answer that you had insurance for the entire year, and you did not have any of the less common types of insurance.

In your MA return indicate that you had insurance that met the MCC all year. You are only required to meet MCC for the time that you lived in MA, if you are filing a part-year resident return. Then you will enter that you had private insurance, and enter the information from your 1099-HC.
