The information comes from a Form 1099-G that the state issued to you. Many states do not mail the 1099-G. You have to access it on the state's taxation web site. If you received a tax refund in 2020 and you did not get a Form 1099-G in the mail, check your state's web site.
Your state/locality may have sent you a 1099-G form (if they haven’t gone paperless). You should receive your 1099-G by mid-February if your state/locality still sends out paper forms. If you did get a 1099-G, make sure it's for a state/local tax refund, not unemployment compensation.

If you received a refund from your 2019 state tax return in 2020, and you claimed the standard deduction (not itemized deductions) on your 2019 federal tax return, you do not have to enter any information about the refund. It does not have to be reported on your 2020 tax return.
